As a Mesa Historical Museum volunteer, I agree:
To give at least 50 hours of volunteer service annually,
To report and log all service hours,
To attend any required training meetings,
To notify the Museum if I am unable to report to work as scheduled,
To become familiar with the Museum, its procedures, and policies, and
To notify my supervisor if any of the information on this form changes.
